People who are homelessness or living with housing instability
Homelessness includes people who are sleeping rough as well as people who are unsafe in their current living arrangements because they are at risk of abuse, “couch-surfing” or are relying informally on someone for accommodation or living in conditions that damage their health such as damp or asbestos.
People can self-present to Launch Housing in Collingwood for assessment around accommodation options on Monday-Friday 9am-5pm. Launch Housing also have a helpline on 1800 825 955 (24/7 toll-free).
The Salvation Army Crisis Contact centre in St Kilda also offers support and accommodation out of hours - their toll free number is 1800 627 727.
Many of our socially vulnerable patients are registered with Cohealth, NRCHC or the Living Room. If follow up or GP review is warranted on discharge from ED consider emailing these GP practices with a discharge summary. There is often a long wait list for new clients.
