ECG of the Week
81yo man with sudden-onset palpitations and SOB. GCS 15. BP 110/60.
1) Describe the ECG.
Monomorphic VT
2) Please list at least 5 differentials for a broad-complex tachycardia.
VT
Torsades de Pointes
Accelerated idioventricular rhythm
SVT with aberrancy
Rapid AF/Atrial flutter with aberrancy
Pacemaker dysfunction
Na channel blocker overdose
Hyperkalaemia
3) What patient and ECG features would make you think of VT rather than other causes of a broad-complex tachyarrhythmia?
4) You diagnose the rhythm as VT. As the patient is currently stable, you consider an amiodarone infusion. Which differentials for broad complex tachyarrhythmia would make amiodarone a potentially catastrophic choice?
Torsades de Pointes
Accelerated idioventricular rhythm
WPW and rapid AF
Na channel blocker toxicity
Hyperkalaemia
